As school nurses, we are responsible for the health and safety of all Woodland students. Feel free to contact us with any questions or concerns that may affect your child's experience at nurse@woodlandschool.org.
An up-to-date Tennessee Certificate of Immunization and Birth Certificate must be submitted prior to the first day of school. Any student without this documentation will be unable to attend class until documents are submitted and reviewed by the school.

Health Forms
A Health History Form and Student Medication Authorization Form are required for all students. Login to the Blackbaud Parent Portal to complete both forms prior to the first day of school.
If any of the following are applicable to your child, please complete along with a physician's signature and return them to the Nurse's office prior to the first day of school.
